In the Community

The Health Market Connect Navigator model was designed using the Asset-Based Community Development method. This means we prioritize partnerships with health centers, hospitals, recovery centers, and other important community stakeholders to help NH residents access affordable health insurance.

HMC convenes the NH Health Coverage Collaborative, a monthly meeting of stakeholders relating to health insurance applications. This includes Navigators, Community Health Workers, Patient Advocates, Brokers, and more. The Health Coverage Collaborative provides a space to share updates and guidance around public health insurance programs.
